Model of Care

Shar Healthcare aims to provide leadership in health education, to promote and empower quality health and wellbeing from the grass roots to senior care facilities.

 

  • Shar Healthcare shall work with PCNs and Care organisations; including local stakeholders and charities, to help deliver a health plan which is proactive, personalised and more integrated with social care.

  • The support provided is evidence based and flexible to fit around the organisations we visit, both face to face to build relationships; but also using safe online technology.

  • Shar Healthcare brings recent health issues to the next generation, such as improving childhood dentistry and menopause in the workplace, sharing good health practice and spreading excellence.
 

Using feedback and the viewpoint of expert patients with successful models of care, we create a holistic approach for our users. Following the pandemic, access to healthcare has been a challenge. Shar Healthcare therefore supports and delivers preventative medicine with joint working, building resilience and improving education on self-care and mental health.

Our programmes on updating training and development on clinical or medicines management projects, such as the ‘Pharmacy First Service’ will support, motivate and inspire current and future healthcare staff. Furthermore, Shar Healthcare embraces healthcare change management; by providing awareness, knowledge and resources to organisations that implement change.
